Calgary Cares Community Services Inc.

Introduction

Welcome to Calgary Cares Community Services Inc. ("CCCS"). We provide client-directed home care services in accordance with Alberta Health Services (AHS) requirements. By engaging our services, you agree to abide by the terms and conditions outlined in this document. These Terms of Service (the "Terms") govern your use of our services.

Definitions

  • Client: The individual receiving home care services from CCCS.
  • CCCS: Calgary Cares Community Services Inc.
  • Service Provider: The professional assigned by CCCS to provide home care services.
  • Services: Home care services provided by CCCS as outlined in the care plan.

Eligibility

Clients must meet the eligibility criteria set by AHS to receive home care services. This includes, but is not limited to, residing within the service area and having a valid Alberta Health Care Insurance Plan (AHCIP) number.

Services Provided

CCCS offers a range of home care services tailored to the needs of each client, which may include:

  • Companion care (e.g., meal preparation, light housekeeping)
  • Personal care (e.g., bathing, dressing, grooming)
  • Nursing care (e.g., medication administration, wound care)
  • Specialty care (e.g. physiotherapy, occupational therapy)

Client Responsibilities

Clients are expected to:

  • Provide accurate and complete health information.
  • Participate in the development and implementation of their care plan.
  • Notify CCCS of any changes in health status or living conditions.
  • Treat CCCS staff with respect and courtesy.
  • Provide a safe environment for the delivery of services.

Service Provider Responsibilities

CCCS and its Service Providers will:

  • Deliver services in a professional, respectful, and compassionate manner.
  • Maintain client confidentiality and privacy.
  • Adhere to the care plan agreed upon with the client.
  • Provide timely and effective communication regarding care.

Privacy and Confidentiality

CCCS is committed to protecting the privacy and confidentiality of client information in accordance with the Health Information Act (HIA) and other applicable laws. Personal health information will be collected, used, and disclosed only for the purposes of providing care and services.

Fees and Payment

Services funded by AHS are provided at no cost to the client. For services not covered by AHS, clients will be informed of any applicable fees prior to service delivery. Payment terms will be agreed upon in advance.

Termination of Services

Services may be terminated by CCCS or the client with reasonable notice. CCCS may terminate services if:

  • The client no longer meets the eligibility criteria.
  • There is a safety risk to the Service Provider.
  • The client consistently fails to comply with the care plan or terms of service.

Complaints and Feedback

CCCS values client feedback and is committed to addressing any concerns. Clients can file complaints or provide feedback by contacting our office directly. Complaints will be handled in accordance with our complaint resolution policy.

Liability

CCCS will not be liable for any direct, indirect, incidental, or consequential damages arising from the use of our services, except where such liability is mandated by law.

Amendments

CCCS reserves the right to amend these Terms of Service at any time. Clients will be notified of any changes in writing.

Governing Law

These Terms are governed by the laws of the Province of Alberta and the applicable federal laws of Canada.
